Cortez Integrated Healthcare, operated by Axis Health System, has been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program.
It is the second year in a row CIH has received this recognition.
Patient-Centered Medical Homes offer many benefits:
It builds strong relationships between patients and their “care team” – a group of providers that collaborates with patients, family members and community service providers to support patient goals.
The program offers care that is safe, effective, patient-centered, timely, efficient and equitable and can lead to lower costs, improved patient experience and better health outcomes.
